In 2019, new vision to expand strategies to align with the goals of the North Carolina State Perinatal Health Strategic Plan was inspired.  After a year of planning and assessment that included implementing a landscape analysis of Guilford County programming, a Collective Action Strategic Planning process was kicked off in 2020 in partnership with Root Cause Institute.  The goal of this process was to identify bold goals and strategies to reduce the immense infant mortality disparities.  By 2026,we collectively will ensure more Black babies are born healthy and live to celebrate their first birthday, because when we create a healthy Guilford County for Black babies then we create a healthy Guilford County for all of us.

Building Transformative Change Requires Radical Inclusion and Collective Action

During the design of the planning phase a local work group was established and developed a partner landscape map using the social-ecological framework.

We believe community transformation requires radical inclusion of diverse partners that are impacted and that impact the health and wellbeing of birthing people and babies in Guilford County.  You can’t get the full picture without all the pieces. You can’t achieve systemic change with our all voices included.

  • Center community and participant voice at key stages of design and throughout the implementation phases
  • Align resources and services related to a specific cause
  • Design and implement shared measurement systems that track outcomes toward overarching goals
  • Design a network with eye toward sustainability
