Public Health Vital Records

The Guilford County Division of Public Health Vital Records Office is responsible for filing all births, deaths, and fetal deaths in Guilford County.

Birth Registration

A certificate of birth must be filed within 10 days with the local health department in the county where the birth occurred, regardless of the gestation period.

Who is responsible for filing the certificate with the local health department? 

The hospital, medical facility, or qualified medical professional is responsible for filing all births with the local health department.   

What information is required to register an intentional home birth?

If your child was born at home in Guilford County, their birth must still be registered within 10 days. If a Certified Nurse-Midwife (CNM) was present for the birth, they will contact the health department. If there was not a CNM in attendance, please visit the below links for information and forms to register an unattended birth outside a medical facility:

One of the following valid, unexpired, government issued documents:

  • Driver’s license
  • State issued photo identification card
  • Passport
  • Permanent residence card (Green Card)
  • Military identification card
  • Other valid picture identification card issued by a foreign government (If the mother gave birth but is not here legally, she might be able to get identification verification from her consulate.
  • If the mother does not possess government-issued identification, the acceptance of the application is at the discretion of the local or deputy registrar

One of the following:

  • Ultrasound that includes the date and mother’s name
  • Medical visit summary (ex. MyChart) that specifies a positive pregnancy test or contains obstetrical data
  • Statement from a licensed healthcare provider who has first-hand knowledge of the pregnancy, and can attest in writing the facts of pregnancy (statement must include mother’s name and DOB, date of last menstrual cycle, or estimated due date- and contain the signature and license number of the clinician)
  • Notarized statement from a non-relative who is a community member and witnessed you pregnant (religious leader, employer, teacher, etc.)
  • Other evidence acceptable to the local health director 

One of the following:

  • Statement from a licensed healthcare provider who saw or examined the infant within the first 30 days of life
  • After visit summary or medical record encounter for a pediatric visit within the first 30 days of life
  • In person visit with the infant at the office of the deputy registrar (appointment only)
  • Other evidence acceptable to the local health director

One of the following:

  • Rental or mortgage receipt dated within 30 days of the birth, showing the address and mother’s name (or father, if he is listed on the birth certificate)
  • Electric power, natural gas, or water bill for the statement period when the birth occurred if the birth occurred at the mother's residence. The copy of the bill (or statement from the company) showing the name of the utility company, address of the residence where the birth occurred, and name of mother or father (if listed on the birth certificate)
  • Letter from a government agency showing mother is receiving services at the address listed
  • If birth occurred elsewhere (relative’s home, non-licensed birth facility, etc.) a notarized statement from the owner/homeowner with proof of their address (ID, current utility bill, letter from a government office, etc.)  is required.

  • This office only processes birth applications for infants under 1 year old. For delayed filings over 1 year, please contact the state vital records office
  • An appointment is required to register your child’s birth
  • A completed mother’s worksheet must be submitted via fax, mail, or email prior to scheduling an appointment.  The deputy registrar may verify the accuracy of all information provided for out-of-facility births
  • Depending on the circumstances of your delivery, you may need to bring a witness who can attest to the birth (18 and older with ID)
  • Depending on the circumstances of your delivery, you may need to bring the infant to the appointment
  • If the parents are not married, the father’s name will not be listed on the birth certificate unless both parents are present to sign a voluntary "Affidavit of Paternity."
  • If mother was married to someone other than the father named on the Affidavit at the time of conception or birth or between conception and birth of a child, the name of the father identified on the Affidavit of Parentage will not be entered on the birth certificate. Per N.C. General Statute 130A-101(e), the husband must be entered as the father on the birth certificate unless all the requirements of N.C. General Statute 130A-101(e)(2) have been met, or if paternity has been otherwise determined by a court of competent jurisdiction (N.C. General Statute 130A-101(e)(1)

How do I add my child’s father to a birth certificate? 

This service is by appointment only. Please call or email for more information. 

Death Registration Information  

The person who first assumes custody of the deceased body is responsible for completing the death certificate in NC DAVE. You do not need to fax an NOD (notification of death).

Burial Transit Permits

How do I obtain a burial transit permit? 

During normal business hours, you must fax or email all requests for a Burial Transit Permit, and the deputy registrar will sign and return it to you. 

Sub-Registrars including the Greensboro Detention Center (Jail) at 201 S. Edgeworth Street, in Greensboro issue all Burial Transit Permits after-hours (evenings, weekends, and holidays).

If the death occurred in a medical facility, they may have a sub-registrar on-site to issue Burial Transit Permits.
