Community Health Programs

Guilford County Division of Public Health Community Services encompasses programs that are community based, meaning services are rendered to residents at places other than the health department. Some of these services are referred to as Home Visiting Services, including: 

Family Connects Guilford

Family Connects Guilford is an evidence-based nurse home visiting program that helps parents better understand how to care for their baby, increase their confidence, provide breastfeeding support, reduce emergency room visits, and reduce the incidence of child abuse and neglect. During a home visit, a Family Connects nurse will address a family’s questions and concerns, offer emotional support, check vital signs/assess the overall health of baby and birthing person, provide education, identify back-to-work and home support and make referrals as needed. Care Management for High-Risk Pregnancy (CMHRP)

The Care Management for High-Risk Pregnancy Program is a free care management service available to pregnant women enrolled in North Carolina Medicaid and to a limited number of uninsured pregnant women with chronic conditions and/or other needs.

Services are provided by trained registered nurses and social workers in the obstetrics practice, home, or other settings. Care managers provide education, encouragement and support based on each individual's need throughout pregnancy and up to 60 days after giving birth.

Community Alternatives Program (CAP/C) for Disabled Children and Disabled and Elderly Adults (CAP/DA)

The Community Alternatives Program (CAP) for Disabled Adults (/DA) and Children (/C) is a statewide program that assists children and adults with Medicaid at home or in the community instead of being institutionalized. Services are provided by trained nurses and social workers. Case management responsibilities include assessments, care planning, regular monitoring, referral, and linkage to other services and supports to address identified needs.

Contacts will vary between in-person, telephone, e-mail, and video-based communication. CAP RNs also help monitor Guilford County Adult Day Health Day Care Centers and occasionally assist with other community health projects.

Care Management for At Risk Children (CMARC)

Care Management for at Risk Children (CMARC) specializes in early childhood comprehensive care management for families with children ages 0-5 with neonatal/chronic health conditions, social determinants of health and developmental delays or exposure to toxic stress/ trauma. Services are provided by specially trained social workers and nurses in the home or various settings. Care managers assist families with developing an individualized plan of care, coordination of community resources, comprehensive needs assessments and basic developmental screenings are also provided.

Child Fatality Prevention Team (CFPT)/ Community Child Protection Team (CCPT)

The Child Fatality Prevention Team (CFPT) was established to review child fatalities and to continually search for ways to prevent these fatalities from occurring. The Community Child Protection Team (CCPT) is a subset of the CFPT and its purpose is to review child deaths attributed to abuse and/or neglect.  Both teams function as one (1) to keep all children safe and advocate for services each family needs to succeed.

Child Care Health Consultants (CCHC) are registered nurses who are formally trained by the North Carolina Health and Safety Resource Center at UNC-Chapel Hill’s North Carolina Institute for Public Health. CCHCs are specialized nurses with skills in community public health, pediatric nursing, and childcare licensing requirements. The CCHC’s work with licensed childcare centers and licensed family childcare homes to achieve high quality, safe and healthy childcare environments. CCHC assists to decrease and/or slow the impact of illnesses in the childcare setting. CCHCs consult with early educators in various ways but are not limited to providing education on ages and stages of development, initiating, reviewing, and revising medical action plans and health policies, assistance with emergency preparedness plans, health and safety assessments, caring for children with chronic illnesses, assistance with immunization records and reviews, referrals, and health and safety trainings to ensure a safe and healthy environment for the early education setting. CCHCs work with early educators, parents and community stakeholders to ensure children are ready to transition from the early childcare environment to school. Our CCHC nurses apply to become certified once qualified, to show our commitment to childcare and health consultation.

Is My Child Eligible to Receive Dental Services?

  • We accept children up to age 21 enrolled in Medicaid or NC Health Choice
  • We accept children who have applied for Medicaid or NC Health Choice, but were declined, whose parents can pay a reduced fee at time of service.

Dental Health Screenings and Education

Guilford Family Connects offers nurse home visits to all deliveries throughout Guilford County. Nurse home visits provide support, education and early identification and referral of health & safety concerns. Nurses with advanced knowledge and experience in public heath, perinatal nursing and lactation support will conduct post-delivery visits with follow-up as needed. Interpreter assistance is provided to Spanish-speaking families, and for other languages on a more limited basis. Visits include a thorough physical assessment of mom and baby, psychosocial assessment of family and environment, referral and follow up of identified needs and educational counseling. Each family visited will receive an educational item and/or digital thermometer when available. GCDHHS follows the Family Connects evidence-based model for implementation of the activity.
