Available Services
Responsible for monitoring Adult Care Homes to ensure licensure compliance, investigation of licensure related complaints. Assists community toward licensure of new adult care homes.
The Adult Day Care/Health Services program provides assistance that allows older adults and adults with disabilities to remain living independently, reduce social isolation, loneliness, and stimulate interest in leisure activities. The program is designated by the state to monitor certification compliance of adult day centers in Guilford County.
The Adult Placement program provides case management services to community members who need support finding a nursing home or adult care home when they are unable to remain in their current living situations.
Through the Guardianship program, social workers provide comprehensive ongoing case management services to adults that have been adjudicated incompetent by the Clerk of Superior Court and for whom the agency director has been appointed legal guardian.

Adult Protective Services provides investigation of suspected abuse, neglect, and exploitation of adults with disabilities. If abuse, neglect, or exploitation is found, we provide services to improve and/or eliminate the situation with adult consent or meet with the legal system when the individual does not have capacity to consent to services.
The Adult Representative Payee Services program provides financial management of SSA/SSI funds for adults who have been determined by the Social Security Administration as being incapable of managing their own benefits.
The Special Assistance In-home program provides Medicaid eligible clients with a special assistance payment that will supplement their income in order to safely remain in their homes. Residents must meet eligibility requirements to qualify for the program.
The In-home Aide Services program contracts with community home care agencies to provide in-home services, when there is a need, such as light housekeeping and personal care services (bathing, hair washing, toileting) for residents safely remaining in their homes.
Offers young adults' opportunities to continue foster care placement and transition to independence with county support. Young adult must be 18 years of age but not yet 21 years of age.
The Adult Resource Team collaborates with local Emergency Medical Services and the Family Justice Center. Individuals identified with high, non-emergency 911 use are partnered with a social worker to identify community service programs to meet health and safety needs. Referrals from EMS/Fire Department only.
Provides eligibility determination for assistance towards cremation services for adult wards and unclaimed bodies. Referrals from hospitals/facilities only.
