School Health

The School Health Program supports Guilford County Schools through case management, health education, and consultation in order to remove health related barriers to learning.

School nurses promote health, safety, and educational success of school age children in Guilford County by serving students, families, and school staff through partnerships, evidence-based practice and professional standards of care.

Students are entering school with many more medical problems than in the past. For example, we now have children in classrooms who receive daily tube feedings. School staff are assisting with medical procedures such as catheterizations, learning to administer insulin to diabetic children and other emergency injections. School nurses provide the medical expertise, the training of teachers and staff, and the monitoring of students with routine and special needs. Guilford County Health and Human Services school nurses collaborate with other human services programs that address substance abuse, mental and physical illness, nutrition, dental and vision needs, obesity, and preventive health care measures.

Health and education leaders recognize that there is a direct connection between health and learning. A healthy student learns better and is better able to meet their academic potential.

The completed form should be returned to your child’s school health nurse or school cafeteria manager.

Medication Authorization Form

If your child needs to take medication while at school, a medication authorization form will need to be completed and submitted to your child’s school. More information is available from Guilford County Schools.

Health Assessment Form

Every child that is new to North Carolina Public Schools, in grades K-12, is required to have a North Carolina Health Assessment Transmittal Form completed by a health care provider and submitted to the school within 30 calendar days of a child’s first day of attendance. This program began during the 2016-2017 school year.

Asthma Education for Parents and Children

The American Lung Association has a number of resources to fit your needs of asthma education for parents and school personnel: Asthma Basics is a 50-minute online learning tool that teaches participants to identify and manage asthma symptoms, understand an Asthma Action Plan, and respond to a breathing emergency.
