 2023 COMMUNITY HEALTH ASSESSMENT

Guilford County Community Health Assessment - Our Community, Our Health 

The Community Health Assessment or CHA is the foundation for improving and promoting community health. Every four years, the Guilford County Division of Public Health engages community partners and members to identify community needs, resources, and strengths.

In this process, we review existing health and community data. We also survey community members and have community conversations to better understand the story of our community’s health.

social determinants of healthThe CHA helps us understand the community’s current well-being and the root causes that keep some residents from having their best health. The 2023 CHA will build on past efforts with focus on health equity and the social determinants of health. These are sometimes called “root causes” or “social drivers of health.” 

This information helps identify pressing health concerns we must work together to address. We then work together to develop community health improvement plans (or CHIPS) to address these concerns. The CHIP describes how a community will work together to address health concerns identified in the CHA, including action steps, implementation, and evaluation.

With your help, we can work together to make Guilford County a healthier place to live, work, and play! Your input is essential!

We appreciate those who shared your lived experiences and how health and community issues impact quality of life in our community. By sharing your perspective, to our Community Health Assessment, you help us to highlight a diverse range of perspectives. The survey data are being analyzed, so stay tuned!  

2023 Guilford County Community Health Survey Update 

In the fall of 2023, we specifically chose 420 households to share their opinions about important health issues and social determinants of health like access to care, transportation, housing, and food insecurity. We would like to thank those who provided feedback in person or virtually!  We would also like to thank the numerous staff and volunteers who assisted with the administration of the survey! The survey data are being analyzed, so stay tuned!
