Home Birth Requirements | Guilford County, NC

In addition to a completed Mother's Worksheet the following is needed:

Proof of Identity of parent(s)

Evidence of Pregnancy

One of the following:

Ultrasound that includes the mother’s name and date
Statement from a licensed healthcare provider who has first-hand knowledge of the pregnancy, and can attest in writing the facts of pregnancy (statement must include mother’s name and DOB, date of last menstrual cycle, or estimated due date- and contain the signature and license number of the clinician)
Notarized statement from a non-relative who is a community member and witnessed you pregnant (religious leader, employer, teacher, etc.)
Other evidence acceptable to the local health director

Evidence of Live Birth

Proof birth occurred in Guilford County

One of the following:

Government issued ID reflecting the mother’s name and current address
Rental agreement or mortgage statement with mother’s name
Utility bill, vehicle registration, W-2 form, or letter from a government agency that mother is receiving services from (must show mother’s name on bill or letter)
*If address does not match ID, two documents with current address is needed

This office only processes birth applications for infants less than 1year old (The state office processes all delayed filings, greater than 1 year
You must have an appointment to register your child’s birth
A completed mother’s worksheet must be submitted via fax, mail, or email prior to scheduling an appointment
The deputy registrar may verify the accuracy of all information provided to register an out-of-facility birth
Depending on the facts of your delivery, a witness who can attest you gave birth may be required to attend the appointment with you
Depending on the facts of your delivery you may be required to bring the infant to your appointment
