GUILFORD COUNTY (NC-504) CONTINUUM OF CARE FORMS
- The Conflict of Interest Disclosure Policy can be accessed here. The NC-504 Guilford County CoC Governance Charter requires the Acknowledgment and Disclosure Statement to be submitted annually by each agency participating in the CoC. A hard copy of this form is available here and should be sent to infoCoC@guilfordcountync.gov.
