If height, weight, hemoglobin, and/or lead are being completed at a physician’s office, the results can be recorded on this form.  WIC Exchange of Information form can be given to the patient to bring to the WIC office or faxed to 336-641-4617 (Greensboro) 336-641-6961 (High Point).  Having this information will reduce time spent in the WIC office and prevent duplication of these services.  We can only use results for a WIC appointment if they are less than 60 days old at the time of the appointment.
