Meet with a WIC Eligibility Caseworker who will verify proof of ID, residence and income

  • To save time, you can upload required documents in advance using our secure online portal.

Be weighed and measured and have lab visit to check your hemoglobin and lead if needed

  • If you have a doctor’s appointment less than 30 days before your WIC appointment, you can have them fill out this document and fax it to our office or this information may be uploaded from your electronic health record (MyChart) using our online portal.

Meet with a Nutritionist who will

  • Evaluate the nutritional needs and desires of each person enrolling
  • Assign a nutrition plan and food package to each person enrolling
  • Review WIC’s services and benefits
  • Teach you how to use your WIC benefits

Meet with a Breastfeeding Peer Counselor, if applicable, who will

  • Discuss the benefits of breastfeeding for mom and baby
Provide support and assistance for breastfeeding
