Each person who is enrolling in WIC must be present.

In addition, you will need to bring the following documents with you:

Identification for anyone who is enrolling (including children age birth to 5)

• Birth Certificate
• Immunization Record
• Social Security Card
• Hospital crib card or ID bracelet
• Photo ID
• Current Medicaid Card or
• Food Stamp Eligibility Letter

A document that verifies your current address

• Utility bill with current address (electric, telephone, gas or water)
• Immunization Record
• Photo ID

If applicable, a paycheck stub, tax return, or letter from your employer verifying your wages

• Check stubs for the most recent 30 days for everyone working in the household
• If you receive Medicaid, SNAP, or Work First benefits, we will not need to verify your wages
